where do we submit our phase 1 report? on the website (where)? or do we email it?

Do you mean your site visit report? You post most of everything through the SMART portal site.

If you are fotunate enough that LMI made you a SMART account, you submit it on the portal. Otherwise, e-mail it to SMART.
